Job summary

Jobgether, on behalf of a partner company, seeks a Telephonic Nurse Care Manager based in the United States. This fully remote role provides nurse-led, patient-centered support to individuals with chronic conditions using virtual assessments, RPM data, and IVR-driven insights.

You will interpret biometric data, develop individualized care plans, coordinate care with interdisciplinary teams, and promote self-management to reduce hospitalizations.

Qualifications

• Associate degree in nursing (ADN) or BSN from an accredited school.
• Active, unrestricted RN license.
• Compact Licensure (NLC) and/or California RN license as applicable.
• Minimum 5 years of broad clinical nursing experience.
• At least 3 years in care management, care coordination, home health, telehealth, or chronic disease management.
• Clinical experience with cardiac, pulmonary, or endocrine conditions is valuable.

Responsibilities

• Conduct telephonic and virtual nursing assessments using biometric, RPM, and IVR data to identify trends and concerns.
• Develop and adjust individualized care plans based on patient needs and protocols.
• Use Motivational Interviewing to support adherence and self-management.
• Identify risks and escalate urgent concerns per guidelines.
• Coordinate referrals and transitions with interdisciplinary teams.
• Deliver proactive nursing interventions to improve outcomes and reduce avoidable visits.
• Maintain accurate documentation in the EHR of all interactions and care activities.
• Contribute to quality improvement initiatives and chart audits.
