Job summary

Private Health Management, Inc. is seeking a Nurse Advisor on the Health Advisory team to guide clients through the healthcare system, educate them, advocate for quality care, and coordinate services. You will support PHM’s triage team and place clients with appropriate internal PHM teams to maximize outcomes.

The role emphasizes clinical expertise, empathy, and a consultative approach, with remote work from anywhere in the United States.

Qualifications

• Active RN license in your state and at least five years healthcare experience.
• Experience in case management, care coordination, patient navigation, ER triage or similar.
• Strong communication, empathy, and clinical judgment.
• Self-directed, collaborative, and comfortable with new technologies.
• Experience working remotely.

Responsibilities

• Manage a caseload of clients delivering clinical support, education, advocacy, care coordination, and ongoing support.
• Perform clinical intake via email or phone to assess needs and plan next steps.
• Collaborate with client services, medical records, and network team to recommend providers and coordinate appointments.
• Provide cross-team support to triage team handling inbound inquiries.
• Communicate clearly and compassionately with clients, caregivers, providers, and internal partners.
