Job summary

Private Health Management, Inc. is seeking a Nurse Advisor for its Health Advisory team. You will manage client portfolios, educate and advocate for patients, and coordinate care with PHM's physicians and staff to improve outcomes.

A nurse with experience in case management and remote work is preferred. This role emphasizes compassionate client engagement, education, and seamless care transitions across PHM’s clinical and operational teams, with opportunities to support onboarding of new clients

Qualifications

• Active RN license in your state and at least five years of healthcare experience.
• Experience in case management, care coordination, patient navigation, ER Triage or a related clinical environment.
• Confidence, empathy, critical thinking, and sound clinical judgment when supporting clients and families.
• Self-directed, collaborative approach with strong organization, time management, and comfort with new technologies.
• Experience working remotely.
• Strong written and verbal communication skills, able to translate complex clinical information for clinical and non-clinical audiences.

Responsibilities

• Manage a caseload of clients delivering responsive transaction-based clinical support including education, advocacy, care coordination and ongoing support.
• Conduct a clinical intake via email or phone to assess needs and determine next steps in care.
• Collaborate with client services coordinators, medical records and network team to provide provider recommendations and coordinate appointments.
• Assist with medical record review and provide clinical guidance when necessary.
• Provide cross-collaboration support to the triage team handling inbound inquiries via email and phone.
• Communicate clearly and compassionately with clients, caregivers, providers, and internal partners.
