Private Health Management is seeking a Nurse Advisor for its Health Advisory team. You will manage a client portfolio, educate clients, advocate for high-quality care, coordinate services, and place clients with PHM teams to optimize outcomes.

This remote role in the United States emphasizes compassionate, consultative support, case management, and collaboration with cross-functional teams to deliver a seamless client experience.

Qualifications

• Active Registered Nurse license in state of residence.
• At least five years of healthcare experience.
• Experience in case management or care coordination.
• Experience with patient navigation and ER triage or equivalent setting.
• Strong written and verbal communication; ability to translate clinical info for non-clinical audiences.

Responsibilities

• Build trusted relationships with clients and guide their healthcare journey.
• Help clients access appropriate providers and services while simplifying care coordination.
• Ensure smooth transitions and communicate across PHM's clinical and operations teams.
• Welcome, educate, triage, and onboard new PHM clients.
• Provide compassionate, high-quality client experience across a portfolio of cases.
