Job summary

Sentara Health in Roanoke, VA seeks an Integrated Case Manager, RN to coordinate care across physical, behavioral health, and social services while traveling to members up to 60-90 minutes several days a week.

The role involves developing and monitoring care plans, performing telephonic assessments, facilitating authorizations, and collaborating with medical directors and interdisciplinary teams to ensure quality, regulatory compliance and cost-effective care.

Qualifications

• Associates degree in nursing or related field.
• Bachelor's degree preferred; RN certification required.
• Minimum of 3 years nursing experience; managed care preferred.

Responsibilities

• Perform telephonic clinical assessments and care coordination.
• Develop and monitor member care plans across care continuum.
• Identify high-risk members and coordinate with health care team.
• Facilitate authorizations/referrals within benefits structure.
• Present cases at case conferences and ensure regulatory compliance.
