Job summary

Sentara Health is seeking an Integrated Case Manager, RN, to deliver comprehensive case management services within licensure, developing and updating member care plans. The role involves telephonic assessments, care coordination, and collaboration with a multidisciplinary team to optimize health outcomes.

Responsibilities include identifying high-risk patients, managing chronic conditions, facilitating authorizations/referrals, and presenting at case conferences.

Qualifications

• Associates degree or higher in nursing with RN licensure.
• Bachelor's degree preferred for this role.

Responsibilities

• Develop and monitor member care plans within licensure scope.
• Perform telephonic clinical assessments to identify, evaluate, coordinate and manage member needs.
• Identify high-risk members and coordinate care with the health care team.
• Manage chronic illnesses, co-morbidities and disabilities to optimize care and cost efficiency.
• Assist with authorizations/referrals within benefits structure or extra-contractual arrangements.
• Interface with Medical Directors and Inter-Disciplinary Teams; present at case conferences; ensure policy compliance.
