Job summary

Enlyte is seeking a Field Case Manager to collaborate with physicians, employers, and patients to develop and manage treatment plans. This full-time, work-from-home role requires travel within Northern Louisiana for in-person appointments and offers a comprehensive benefits package.

You will assess recovery, modify job duties, and ensure cost-effective healthcare services while maintaining high standards of care and client satisfaction.

Qualifications

• Education: Associates Degree or Bachelor's Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred. Workers’ compensation-related experience preferred.
• Skills: Advocate recommendations effectively; work independently; basic computer skills including Excel, Word, and Outlook Email; proficient grammar and written communication.

Responsibilities

• Apply case management standards of practice.
• Use knowledge of injuries, medications, comorbidities and treatment options to advise on plans.
• Interview disabled persons to assess recovery and determine occupational relevance.
• Collaborate with treating physicians to tailor treatment plans.
• Work with employers and physicians to modify job duties for early return to work.
• Evaluate goals based on patient progress and treatment effectiveness.
• Manage workload independently and prioritize cases.
• Attend appointments and prepare status updates as needed.
