Acentra Health seeks a Clinical Manager (RN) to provide clinical and operational leadership for CA UIS Care Coordination and Case Management programs. The role drives quality, engagement, and contract performance while overseeing clinical staff and operations.

This is a remote position within California with a preference for Sacramento, LA, or San Diego areas, requiring up to 25% travel. Strong leadership, communication, and regulatory compliance are essential to succeed.

Qualifications

• Active, unrestricted RN license (state or compact) required.
• Associate degree or 7 years of equivalent nursing/healthcare experience.
• 5+ years of healthcare management or leadership experience.
• Excellent verbal and written communication skills.
• Willingness to travel up to 25% within CA.

Responsibilities

• Provide clinical and operational leadership for Care Coordination and Discharge Case Management.
• Oversee Case Managers and Care Coordinators, including coaching and accountability.
• Ensure program deliverables and contract requirements are met.
• Monitor KPIs, quality metrics, and drive process improvements.
• Partner with leadership to identify opportunities for efficiency.
• Foster accountability and high-quality member service.
• Comply with HIPAA and corporate policies.
