Job summary

Providence Health and Services is seeking a Clinical Documentation Specialist RN for our Home Services Oregon team. This role ensures quality review of OASIS admission documents and Plans of Care to maintain CMS compliance in home health settings, collaborating with management to optimize patient outcomes and reimbursement.

The position requires an active RN license, home health experience, and proficiency with electronic medical records, with a remote schedule on weekdays.

Qualifications

• Active RN license in Oregon.
• 2 years nursing experience in Home Health/Palliative Care.
• Experience with an EMR system.
• Must be licensed in states where care is delivered.

Responsibilities

• Quality review of OASIS admission documents and Plans of Care to ensure CMS program compliance.
• Collaborate with Manager of Performance and Home Health leadership to promote regulatory compliance and patient outcomes.
• Serve as Oasis expert to Home Health staff across the region and support reimbursement improvements.
• Promote best practices and ensure adherence to home health regulatory standards.
