Job summary

Premera Blue Cross is seeking a Telecommuter Case Manager to support members with whole-person care. You will conduct holistic assessments, identify social determinants, and collaborate with caregivers/providers to create and monitor member-centric plans.

Requirements include RN licensure and direct care/case management experience; CCM certification is preferred and encouraged to obtain within 24 months. Hybrid training and extensive total rewards accompany the role.

Qualifications

• Active WA State or Compact RN or ARNP license that allows independent assessment.
• 3 years' post licensure direct care experience.
• 1 year of post-licensure case management experience (care coordination/planning/management).
• Preferred CCM certification; obtain within 24 months if not present at hire.
• Bachelor’s or Master’s degree; multiple certifications encouraged.

Responsibilities

• Engage members facing medical utilization disorders and build trusting relationships.
• Conduct holistic health assessments noting social determinants of health and cultural influences.
• Collaborate with caregivers and providers to develop member-centric care plans.
• Monitor and update case management plans to ensure progress and relevance of goals.
• Coordinate communications among health care team to ensure appropriate care delivery.
• Provide interventions to remove barriers and promote behavior change using motivational interviewing.
• Maintain timely, accurate documentation meeting accreditation and contractual requirements.
