Job summary

Premera Blue Cross seeks a Case Manager to support members through a whole-person care model. This telecommuter role partners with clinicians to identify barriers, empower progress, and coordinate providers toward better health.

You will conduct holistic health assessments, address social determinants of health, and create member-centric plans. Training and a comprehensive rewards package accompany the role.

Qualifications

• Active WA State or Compact RN/ARNP license enabling independent assessment.
• 3 years post-licensure direct care experience.
• 1 year post-licensure case management experience.
• CCM certification preferred or obtain within 24 months of hire.
• Bachelor’s or Master’s degree in a relevant field.
• 4+ years direct care across multiple patient populations or settings.
• Motivational interviewing experience and related certifications are a plus.

Responsibilities

• Engage members facing medical utilization disorders and build a relationship for open dialogue on sensitive topics.
• Conduct initial holistic health assessments and identify social determinants of health and cultural influences.
• Create a space where case managers, members, caregivers and providers develop a member-centric plan with goals and barriers.
• Monitor and update the effectiveness of the case management plan toward progress.
• Facilitate communication and coordination among the health care team to maximize appropriate care.
• Provide interventions to remove barriers and promote positive behavior change.
• Maintain concise, timely documentation compliant with accreditation, contractual, and legal requirements.
