Job summary

Premera Blue Cross is seeking a Telecommuter Case Manager to support members through a whole-person care model. You will conduct holistic health assessments, collaborate with caregivers and providers, and guide care plans that promote self-care and better outcomes, while working remotely from your home office.

The role requires WA licensure, 3+ years of direct care, and 1+ year in case management, with opportunities for training, licensure renewal support, and professional certification pathways.

Qualifications

• Active WA State or Compact RN or ARNP license that allows independent assessment.
• 3 years' post licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time.

Responsibilities

• Engage members facing medical utilization disorders and build a relationship for open dialogue.
• Conduct initial holistic health assessments and identify social determinants of health.
• Create a space for collaborative case management plans with members and providers.
• Monitor and update the effectiveness of the care plan toward goals.
• Facilitate communication among the health care team to optimize care delivery.
• Provide interventions to remove barriers and promote positive behavior change.
