Premera Blue Cross is seeking a Case Manager to support members through a whole-person care model as a telecommuter. You will assess needs, coordinate care, and help set attainable goals with a focus on improving health outcomes, while collaborating with caregivers and providers.

The role requires an active WA license, post-licensure care experience, and the ability to work a full-time schedule with some extended hours; training and total rewards are provided.

Qualifikationen

• Active WA State or Compact RN/ARNP license to conduct assessments independently.
• 3 years' post-licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination or discharge planning.
• Bachelor’s or Master’s degree is preferred.
• CCM certification is preferred; obtain within 24 months if not held at hire.
• LEAN training is a plus.

Aufgaben

• Engage members facing medical utilization disorders and build a relationship for open dialogue.
• Conduct initial holistic health assessments to identify social determinants of health and cultural influences.
• Develop member-centric case management plans with caregivers and providers.
• Monitor and update the plan to ensure progress toward goals.
• Facilitate communication among the health care team to maximize care delivery.
• Provide interventions for removing barriers and promoting behavior change using motivational interviewing.
• Maintain concise, accurate documentation meeting accreditation and contractual requirements.
