Qualifications

• Active WA State or Compact RN/ARNP license required.
• Minimum 3 years post-licensure direct care experience.
• Minimum 1 year post-licensure case management experience.
• CCM certification required or obtained within 24 months of hire.
• Bachelor’s or Master’s degree in a related field or equivalent experience.
• Motivational interviewing experience desired.
• Chronic Condition Certifications (Asthma, Cardiology, Diabetes, etc.) required.
• Integrated Case Management certificate (ICM) desired.
• CDP certification encouraged.
• LEAN training preferred.

Responsibilities

• Engage members and build rapport to support open, honest dialogue.
• Perform initial holistic health assessments including SDOH and cultural factors.
• Collaborate with member, caregivers, and providers to develop a member-centric plan.
• Monitor and update progress toward goals and adjust as needed.
• Facilitate communication among the health care team to ensure appropriate care.
• Provide interventions to remove barriers and support behavior change.
• Maintain accurate documentation meeting accreditation and contractual requirements.
