Job summary

Premera Blue Cross seeks a skilled Case Manager to support members through a whole-person care model. This telecommuter role focuses on assessing needs, removing barriers, coordinating care, and guiding members toward healthier daily living.

You will conduct holistic health assessments, identify social determinants of health, collaborate with caregivers and providers, and document progress to meet accreditation and contractual requirements. Training and career development are emphasized.

Qualifications

• Active WA State or Compact RN or ARNP license required.
• Minimum 3 years of post-licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination, discharge planning, or management of patient care over time

Responsibilities

• Engage members facing medical utilization disorders and build trust.
• Conduct holistic health assessments to identify social determinants of health (SDOH) and cultural influences.
• Collaborate with caregivers and providers to develop a member-centric case management plan that sets goals and addresses barriers.
• Monitor and update the care plan to ensure progress toward goals and relevance of current goals.
• Facilitate communication among the health care team to maximize delivery of appropriate care.
• Provide interventions focused on removing barriers and promoting positive behavior change.
