Job summary

Premera is hiring a Case Manager, RN for a telecommuter role. You will assess members, develop and monitor care plans, and coordinate with health care providers to remove barriers and improve outcomes. Requires WA state RN license and 3+ years direct care and 1 year case management experience.

The role offers substantial training, opportunities for certifications, and a comprehensive benefits package. Training hours are 8-5 PT with occasional extended hours to cover after-hours calls.

Qualifications

• Active WA State nursing license or compact license.
• 3+ years post-licensure direct care experience.
• 1 year post-licensure case management experience.
• If CCM not yet obtained at hire, must obtain within 24 months of hire.

Responsibilities

• Engage members facing medical utilization disorders and build a relationship that supports open dialogue.
• Conduct holistic health assessments to identify SDOH and cultural influences affecting lifestyle changes.
• Create a member-centric case management plan with caregivers and providers.
• Monitor and update the plan to ensure progress toward goals is maintained.
• Facilitate communication among the health care team to maximize appropriate care.
• Provide interventions to remove barriers and promote positive behavior change using motivational interviewing.
• Maintain accurate, timely documentation meeting accreditation and legal requirements.
