Premera Blue Cross is seeking a Telecommuter Case Manager to support members through a whole-person care model. You will conduct holistic assessments, identify barriers, and collaborate with caregivers and providers to develop member‑centric plans that improve health outcomes.

The role emphasizes coordination, motivational interviewing, and continuous plan evaluation while maintaining compliant documentation and a flexible, hybrid work setup.

Qualifications

• Active WA State or Compact RN or ARNP license that allows you to conduct an assessment independently.
• 3 years' post licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time.
• CCM certification preferred; if not present at hire, must obtain within 24 months.
• Bachelor’s or Master’s degree requested.

Responsibilities

• Engage members facing medical utilization disorders and build a constructive dialogue.
• Conduct initial holistic health assessments and identify social determinants of health.
• Create a space for collaboration to develop a member-centric case management plan.
• Monitor and update the plan to ensure progress toward goals.
• Facilitate communication among the health care team to maximize appropriate care.
• Provide interventions to remove barriers and promote positive behavior change.
• Maintain concise documentation meeting accreditation and legal requirements.
