Job summary

Premera Blue Cross is hiring for a telecommuter Case Manager who supports members through a whole-person care model. You will assess needs, identify social determinants of health, and collaborate with caregivers and providers to develop a member-centric plan.

You will monitor progress, coordinate care, and document thoroughly while benefiting from extensive training and a generous total rewards program, including tuition assistance and commuter perks.

Qualifications

• Active WA State or Compact RN or ARNP license allowing independent assessment.
• 3 years' post-licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time.
• CCM certification preferred; Bachelor’s or Master’s degree preferred; 4+ years direct care across multiple populations or care settings; Motivational interviewing experience; Chronic Condition Certifications; Integrated Case Management certificate (ICM); Chemical Dependency Professional (CDP) certification; LEAN training.

Responsibilities

• Engage members facing medical utilization disorders and build a relationship that supports open and constructive dialog on sensitive topics.
• Conduct initial holistic health assessments to identify social determinants of health and cultural influences.
• Create a space for collaboration to develop a member-centric case management plan with goals and barriers.
• Monitor, evaluate, and update the case management plan to ensure progress and relevance of goals.
• Facilitate communication among the health care team to maximize appropriate care.
• Provide interventions to remove barriers and promote positive behavior change using motivational interviewing.
• Maintain concise, accurate, and timely documentation that meets accreditation, contractual, and legal requirements.
