Job summary

Premera Blue Cross is seeking a telecommuter Case Manager to support members through a whole-person care model. You will assess, plan, and coordinate care with clinicians, caregivers, and providers to improve health outcomes and remove barriers, all within a remote, national framework.

The role emphasizes motivational interviewing, ongoing plan evaluation, and collaboration with the health care team, with comprehensive total rewards and training opportunities provided.

Qualifications

• Active WA State or Compact RN or ARNP license that allows independent assessment.
• 3 years' post-licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time.
• Preferred: CCM certification; if not present at hire, must obtain within 24 months.
• Bachelor’s or Master’s degree.
• 4+ years direct care experience across multiple patient populations, conditions, or care settings.
• Motivational interviewing experience.
• Integrated Case Management certificate (ICM) or related certifications may be considered.

Responsibilities

• Engage members facing medical utilization disorders and build a relationship for open dialogue.
• Conduct initial holistic health assessments addressing social determinants of health and cultural influences.
• Create a member-centric case management plan with caregivers and providers.
• Monitor and adjust the case management plan to ensure goal progress.
• Facilitate communication among the health care team to maximize appropriate care.
• Provide interventions to remove barriers and promote behavior change.
• Maintain concise documentation meeting accreditation and contract requirements.
