Job summary

CVS Health is seeking an experienced Registered Nurse Case Manager to provide telephonic case management for members with complex healthcare needs. This remote role involves assessing, planning, coordinating, and managing care to improve health outcomes and navigate resources.

The ideal candidate holds an active RN license, at least 5 years of clinical RN experience, and ADN. Compensation includes a base pay range with eligibility for CVS Health bonuses and benefits.

Qualifications

• Active RN license in state of residence.
• Willingness to obtain Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• ADN required; BSN preferred.
• Ability to work Monday–Friday business hours.

Responsibilities

• Conduct telephonic assessments and develop care plans.
• Identify health risks and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to clinical programs.
• Apply clinical judgment to support members with complex conditions.
• Collaborate with multidisciplinary teams to achieve outcomes.
• Maintain compliance with regulatory requirements and policies.
