CVS Health Corporation is seeking an experienced Registered Nurse Case Manager to provide telephonic case management for members with complex healthcare needs. You will assess, plan, coordinate, and manage care while helping members navigate resources and improve health outcomes.

The role requires an active RN license, ADN, and at least five years of clinical RN experience, with a Monday–Friday schedule and a remote work option.

Qualifications

• Active, unrestricted RN license in state of residence.
• Willingness to obtain a Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Ability to work within Monday-Friday business hours.

Responsabilités

• Conduct telephonic case management assessments and develop care plans.
• Identify health risks, barriers, and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to clinical programs and resources.
• Apply clinical judgment to support members with complex and co‑morbid conditions.
• Collaborate with multidisciplinary teams to achieve optimal member outcomes.
