Job summary

CVS Health Corporation is seeking an experienced Registered Nurse Case Manager to provide telephonic, remote case management for members with complex healthcare needs. You will assess, plan, coordinate, and manage care while helping members navigate resources and improve health outcomes.

Join a multidisciplinary team within a full-time, Monday–Friday schedule with 40 hours. CVS Health offers comprehensive benefits and support for licensing needs, including company-paid Compact RN license

Qualifications

Active, unrestricted RN license in state of residence.
Willingness to obtain a Compact RN license (company-paid).
Minimum 5 years of clinical RN experience.
Associate Degree in Nursing (ADN).
Ability to work within Monday-Friday business hours.

Responsibilities

Conduct telephonic assessments and develop individualized care plans.
Identify health risks, barriers, and opportunities to improve member wellness.
Coordinate care, referrals, and transitions to appropriate clinical programs and resources.
Apply clinical judgment to support members with complex and co-morbid conditions.
Collaborate with multidisciplinary teams to achieve optimal member outcomes.
Maintain compliance with regulatory requirements and company policies.
