Job summary

CVS Health is seeking an experienced Registered Nurse Case Manager to provide telephonic support for members with complex healthcare needs. You will assess, plan, coordinate, and manage care while helping members navigate resources and improve health outcomes.

The role offers a remote, full-time schedule with Monday–Friday hours and a competitive benefits package. A valid RN license, ADN, and 5 years of clinical RN experience are required, with company-paid Compact RN licensure encouraged.

Qualifications

• Active, unrestricted RN license in state of residence.
• Willingness to obtain a Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Associate Degree in Nursing (ADN) required.
• Ability to work within Monday-Friday business hours.

Responsibilities

• Conduct telephonic case management assessments and develop care plans.
• Identify health risks and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to programs and resources.
• Apply clinical judgment to support members with complex conditions.
• Collaborate with multidisciplinary teams to achieve outcomes.
• Maintain regulatory and policy compliance.
