Job summary

CVS Health in the Southeast region seeks a Community Care Case Manager to lead patient-centered care coordination. You will collaborate with members, providers and community resources to promote wellness, facilitate appropriate benefits, and support timely return to work.

You will assess needs, plan care, and document activities while maintaining HIPAA compliance and working within a multidisciplinary team to improve outcomes.

Qualifications

• Active and unrestricted Compact RN license in the Southeast region.
• 3+ years clinical practical experience with preference in chronic conditions.
• Experience with case management, discharge planning or home health coordination.
• Ability to obtain multi-state RN licenses if required.

Responsibilities

• Act as liaison among members, providers, and insurers to coordinate care.
• Coordinate case management for diverse patient populations across the continuum.
• Interact with members via telephone or in person to assess needs and plan care.
• Document case work activities and collaborate with multidisciplinary teams.
• Educate on benefits, wellness strategies and eligibility requirements.
• Ensure compliance with laws, regulations and HIPAA guidelines.
