CVS Health is seeking an experienced Case Manager (RN) for remote roles in the Southeast region. You will collaborate with members, providers, and care teams to coordinate benefits, assessments, and care plans tailored to individual needs.

Responsibilities include evaluating eligibility, promoting wellness, and ensuring timely return to work while maintaining HIPAA compliance. A valid RN license and strong communication skills are essential.

Qualifikationen

• 3+ years clinical experience in diabetes, CHF, CKD, post-acute, hospice, cardiac or home health.
• 2+ years case management or discharge planning in relevant settings.
• Active and unrestricted Compact RN license; willingness to obtain multi-state licenses.
• Associate's Degree in Nursing is required; BSN preferred.

Aufgaben

• Coordinate case management for catastrophic and chronically ill members.
• Interact with members, providers, insurers via phone or in person.
• Assess medical and vocational status; plan care to optimize outcomes.
• Document case work and communicate with multidisciplinary team.
• Educate on health strategies; promote preventive care and wellness.
• Ensure compliance with laws, regulations, and insurer requirements.
