Job summary

CVS Health in Raleigh, NC seeks an experienced RN Case Manager to elevate patient care by coordinating comprehensive case management across the continuum of care. You will engage members and families, collaborate with providers and insurers, and drive care plans to optimize wellness and timely return to baseline.

The role requires active RN licensure, 3+ years of clinical practice, and 2+ years in case management or discharge planning.

Qualifications

• Active and unrestricted RN licensure in applicable states
• 3+ years of clinical practice preferred (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, Home Health)
• 2+ years of case management, discharge planning or home health coordination experience
• Proficiency with Microsoft Word, Excel, Outlook and PowerPoint; ability to navigate multiple systems

Responsibilities

• Liaison with members, families, employers, providers, and insurers
• Coordinate case management activities for catastrophic and chronically ill cases
• Interact with members telephonically or in person, including home visits as needed
• Assess medical and vocational status; develop care plans to optimize wellness and outcomes
• Document case work activities and communicate with stakeholders
• Consult with internal multidisciplinary team to maximize health outcomes
• Outreach to treating physicians or specialists as appropriate
• Provide education and preventive information for better outcomes
