Job summary

CVS Health in Massachusetts is seeking an experienced RN to join our Community Care team. You will coordinate case management for Medicare members across care settings, collaborate with providers and families, and navigate resources to improve wellness and outcomes.

Qualifications

• Must reside in MA and hold MA RN licensure.
• 3+ years of clinical practice with Medicare members preferred (diabetes, CHF, CKD, etc.).
• 2+ years of case management, discharge planning or home health coordination.
• Proficient with Microsoft Word, Excel, Outlook and PowerPoint; strong multitasking skills.

Responsibilities

• Acts as liaison with member, employer, provider, insurance and healthcare personnel.
• Coordinates case management for catastrophic and chronically ill members across care continuum.
• Interacts with members telephonically or in person, including home visits.
• Assesses medical and vocational status; develops care plans to optimize wellness and outcomes.
• Communicates with stakeholders and prepares documentation of case work.
• Consults with multidisciplinary team to maximize health outcomes.
• Outreach to treating physicians or specialists as needed.
• Provides educational and prevention information to support medical outcomes.
