CVS Health is seeking an experienced RN Case Manager to join our Community Care team. You will collaborate with members, providers, and care partners to coordinate comprehensive case management across the continuum of care, including home visits and telephonic support.

In this role, you will assess patient needs, develop care plans, monitor progress, and ensure timely return to baseline. Requirements include active RN licensure in NC (or compact state), 3+ years of clinical practice, and 2+

Qualifications

• Active and unrestricted RN licensure (compact in listed states or NC).
• 3+ years clinical practice; preference in diabetes, CHF, CKD, post-acute, hospice, palliative, cardiac, Home Health for Medicare members.
• 2+ years in case management, discharge planning, or home health coordination.

Responsabilités

• Coordinate case management activities for high-need members across care continuum.
• Interact with members telephonically or in person; provide home visits if needed.
• Develop and manage care plans; assess needs and eligibility for benefits.
• Document case work activities; collaborate with internal teams and providers.
