Job summary

Acentra Health in the United States is seeking a Nurse Case Manager to join our growing team. The role evaluates beneficiaries' needs, develops individualized care plans, and coordinates care across the health team to meet payer requirements.

This is a remote position requiring CA residency (Los Angeles or San Diego area preferred). You will handle caseloads, document assessments in our web-based system, and uphold HIPAA and other regulations while pursuing ongoing education.

Qualifications

• Bachelor’s degree/BSN required.
• Active, unrestricted RN license in CA.
• Minimum 3+ years of clinical experience.
• Minimum 1+ year of case management review experience.
• Must reside in CA.
• Strong written/verbal communication and customer service.
• Ability to work independently and as part of a team.
• Maintain a high level of confidentiality.
• Ability to meet department metrics and goals.

Responsibilities

• Assess, plan, implement, monitor, and evaluate care across the continuum.
• Coordinate with the healthcare team to develop beneficiary-centered care.
• Maintain client confidentiality and comply with regulations.
• Document assessment results in the web-based database timely.
• Pursue ongoing education and stay current with case management standards.
• Read and adhere to corporate policies including HIPAA.
