Acentra Health is seeking a Nurse Case Manager to join our growing team, with the role remote for candidates residing in California. You will evaluate beneficiaries' needs, develop individualized care plans, and coordinate healthcare team members to ensure optimal outcomes and compliance with payer requirements.

The position requires a Bachelor's degree/BSN and an active CA RN license, with at least 3 years of clinical experience and 1 year in case management.

Qualifications

• Bachelor’s degree or BSN required.
• Active, unrestricted RN license in CA.
• Minimum 3+ years of clinical experience.
• Minimum 1+ year of case management experience.
• Must reside in CA and be able to work remotely from CA.

Responsibilities

• Assess, plan, implement, monitor, and evaluate care across the continuum.
• Develop beneficiary-centered care plans and coordinate with healthcare teams.
• Maintain confidentiality and comply with HIPAA and related policies.
• Pursue ongoing education and stay current with regulatory changes.
• Read and adhere to corporate policies and payer requirements.
