Job summary

Acentra Health is seeking a Case Manager (LPN or RN) to join our growing team. This remote role requires residence in Oregon and focuses on providing coordinated, patient-centered care to beneficiaries across the state.

You will coordinate care across social and health services, engage with a multidisciplinary team, educate beneficiaries about resources, and document assessments to meet regulatory standards and improve outcomes.

Qualifications

• Requires Oregon RN or LPN license and 2+ years Medicaid-funded case management or care coordination.
• Experience with diverse populations and health disparities is preferred.
• Knowledge of HIPAA and regulatory requirements.

Responsibilities

• Provide care coordination/care management for beneficiaries with strong clinical knowledge.
• Collaborate with multidisciplinary care team to meet patient needs.
• Complete health and social assessments for care planning using motivational interviewing and equity principles.
• Maintain documentation and PHI security per policies and procedures.
• Monitor caseload and ensure timely entry of assessments into web-based database.

Education

Associate's degree or practical nursing diploma
