Job summary

CVS Health is hiring a Case Manager RN for a 100% remote role. You will assess, plan, facilitate and advocate for members to meet benefit plans and health needs. Work in collaboration with Medical Directors, manage discharge planning, and coordinate care across settings from your home state.

The role requires an active RN license, 3+ years of acute care RN experience, and the ability to work 8:00am–4:30pm in the resident time zone with occasional late shifts. Travel is not required.

Qualifications

• Active RN license in home state.
• 3+ years acute care RN experience.
• Willingness to obtain additional state licenses upon hire (paid by the company).
• Remote work 100%.

Responsibilities

• Apply data-driven methods to identify members and tailor case management programs or referrals.
• Conduct comprehensive clinical assessments.
• Evaluate needs and develop flexible approaches based on member needs and benefit plans.
• Advocate for patients to maximize health care coverage.
• Promote quality and cost-effective outcomes and suggest improvements.
• Identify and elevate quality of care issues through established channels.
• Provide high level of customer service and support.
• Assess health literacy and readiness to change; adapt approaches accordingly.
• Use motivational interviewing to maximize member engagement and lifestyle changes.
• Provide coaching and information to empower independent health decisions.
