CVS Health is seeking a compassionate Case Manager RN to support members through personalized, high-quality care coordination in a fully remote role nationwide. You will conduct assessments, develop and monitor care plans, assist with discharge planning, and advocate for benefits while collaborating with care teams and Medical Directors.

Required: active RN license and 3+ years acute care; BSN preferred; remote work and state licensure flexibility offered.

Qualifications

• Active, unrestricted RN license in your state of residence.
• 3+ years of acute care RN experience (Med‑Surg or Critical Care preferred).
• Willingness to obtain additional state licenses (company paid).

Responsabilités

• Conduct comprehensive clinical assessments and identify care coordination needs.
• Develop, implement, and monitor individualized case management plans.
• Support inpatient review and discharge planning as appropriate.
• Advocate for members to maximize benefits and access appropriate services.
• Engage members through coaching, education, and motivational interviewing.
• Identify and elevate quality‑of‑care concerns.
• Deliver exceptional customer experience while collaborating with care teams and Medical Directors.
