CVS Health is seeking an experienced Case Manager RN for a 100% remote role, with preference for Massachusetts residents and Compact license holders. The role requires EST local time availability and a 40-hour work week, with standard M-F hours and flexible time to meet member needs.

Responsibilities include proactive care planning, evaluating member needs, and coordinating plan benefits to improve outcomes, while ensuring compliance with regulatory policies and maintaining quality care

Qualifications

Active and unrestricted RN license in state of residence, preferably compact.
RN living in EST Time Zone.
Willingness and ability to obtain additional state licenses upon hire (paid by company).
3+ years of acute care clinical experience as an RN (general medical, post-surgical, pediatrics, ICU, case management and discharge planning).
0-10% travel may be required.

Responsibilities

Collaboratively develops a proactive course of action to address issues and enhance member wellness.
Evaluates member needs and benefits to reference plan sponsor programs.
Applies clinical judgment to reduce risk factors and address complex health issues.
Uses case management processes in line with policies and regulations.
Interviews members to assess health status and needs.
