Job summary

CVS Health is seeking an RN Case Manager for a 100% remote position. You will telephonically and face-to-face assess, plan, implement, and coordinate case management activities to evaluate the medical needs of members and facilitate overall wellness.

Required are an active RN license, 3+ years of acute care nursing, and the ability to work 8-hour shifts in their time zone. Home office requirements include a private workspace and reliable internet, with occasional on-site travel as needed.

Qualifications

• Must have an active current and unrestricted RN license in state of residence.
• Compact RN licensure is preferred but not required.
• 3+ years of acute care clinical experience as an RN (medical, post-surgical, ICU, discharge planning).
• Ability to use a computer with multiple screens and sit for extended periods.
• Private designated workspace with high-speed internet.

Responsibilities

• Assess, plan, implement, and coordinate all case management activities with members telephonically and face-to-face.
• Collaborate to refer to programs offered by the plan sponsor and evaluate benefits.
• Apply clinical judgment to reduce risk factors and address health issues.
• Follow regulatory and company policies in case management processes.
• Engage members to assess health status and needs through interviewing.
