Job summary

CVS Health is seeking a Case Manager RN for a 100% remote role. You must hold an active RN license in your state and have at least 3 years of acute care experience. Work hours align with a weekday schedule, with occasional late shifts, and no travel is expected.

You will assess health needs, coordinate care, develop and manage case plans, and advocate for patients within existing benefits. HIPAA-compliant, patient-centered care is essential.

Qualifications

• Must have active RN license in state of residence.
• 3+ years acute care RN experience required.
• Willingness to obtain additional state licenses upon hire (paid for by CVS Health).

Responsibilities

• Assess member health status and care coordination needs.
• Develop and implement the case management plan.
• Advocate for patients to maximize health care coverage.
• Promote quality and cost-effective outcomes; identify care issues.
• Collaborate with Medical Director as indicated; close cases when discharge criteria are met.
