Job summary

CVS Health is seeking an RN Case Manager to work remotely. You will telephonically and in some cases face-to-face assess, plan, implement, and coordinate case management to evaluate medical needs and promote overall wellness.

Applicants must hold an active RN license in their state of residence; multiple-state licensure is welcome. Normal hours are Mon–Fri 9:00am–5:30pm in the resident time zone, with occasional evenings, weekends, and holidays.

Qualifications

• Active RN license in state of residence.
• 2+ years of clinical RN experience; various settings eligible.
• Willingness to obtain multi-state licensure if needed.

Responsibilities

• Assess, plan, implement and coordinate case management activities.
• Evaluate medical needs to support member wellness.
• Use clinical judgment to reduce risk factors and barriers.
• Review prior claims and eligibility for case management.
• Conduct holistic assessments including work capacity and restrictions.
• Collaborate with supervisor and teams; present cases as needed.
• Adhere to regulatory and CVS Health policies; ensure member engagement.
