CVS Health is seeking a compassionate Case Manager RN for a 100% remote, nationwide role. You will coordinate care, perform assessments, and advocate for members to optimize outcomes while collaborating with Medical Directors and care teams.

This full-time position emphasizes patient-centric care, strong clinical judgment, and the ability to work across states with company-paid licensure support. Eligible for CVS Health bonuses and comprehensive benefits.

Qualifications

• Active, unrestricted RN license in your state of residence.
• Willingness to obtain additional state licenses (company paid).
• 3+ years of acute care RN experience (Med-Surg or Critical Care preferred).
• Strong clinical judgment, decision-making, and organizational skills.
• Associate Degree in Nursing is required; BSN preferred.

Responsabilités

• Conduct comprehensive clinical assessments and identify care coordination needs.
• Develop, implement, and monitor individualized case management plans.
• Support inpatient review and discharge planning as appropriate.
• Advocate for members to maximize benefits and access appropriate services.
• Engage members through coaching, education, and motivational interviewing.
• Identify and elevate quality-of-care concerns.
• Deliver exceptional customer experience while collaborating with care teams and Medical Directors.
