Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) to support members from home in a seasonal, work-from-home capacity. The role centers on telephonic health assessments, personalized care plans, and ongoing coaching for Medicare, Medicaid, and group plans.

You will apply nursing judgment to coordinate interventions, support transitions of care, and help members navigate benefits, while collaborating with a multidisciplinary team. RN license and remote work ability required.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• 2+ years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise nursing judgment remotely.
• Intermediate to advanced computer proficiency, including MS Word, Excel, Outlook.
• Ability to navigate multiple electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate healthcare benefits, services, and options.
• Assess factors that may affect a member's health and well-being.
• Collaborate with multidisciplinary teams including social workers, dietitians, pharmacists, providers.
• Promote self-management by providing education and resources.
• Coordinate care activities among members, caregivers, providers, and care team.
• Document interactions, assessments, care plans, interventions, and outcomes accurately in systems.
