Sagility is seeking a Senior Care Specialist (Registered Nurse) to join our work-from-home team in Ohio. This seasonal opportunity offers remote work while delivering telephonic health assessments and care coordination for Medicare, Medicaid, and group health plan members.

You will develop individualized care plans, provide health coaching, and collaborate with multidisciplinary teams to promote safe transitions of care and optimal outcomes.

Qualifikationen

Active, unrestricted RN license in the state of residence.
Compact RN license permitting practice in applicable compact states.
Minimum of 2 years clinical nursing experience; 3+ years preferred.
Ability to work independently in a remote environment.
Medium to advanced computer proficiency (Word, Excel, Outlook).
Ability to navigate multiple EMR systems.

Conduct telephonic health assessments for members.
Develop actionable, measurable care plans.
Establish health coaching relationships and provide education.
Assess needs and coordinate appropriate interventions.
Support transitions of care and discharge planning.
