Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) to work from home in a seasonal assignment. You will conduct telephonic health assessments, develop care plans, and coach members on chronic condition management.

This remote role requires strong nursing judgment, the ability to navigate EMR systems, and collaboration with a multidisciplinary team. Starting pay is $37.00 per hour, with daily pay options and flexible home-based scheduling.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Proficiency with Microsoft Word, Excel, and Outlook; navigate multiple EMR systems.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on needs and risks.
• Establish health coaching relationships and provide education and support.
• Use nursing judgment to assess needs and coordinate appropriate interventions.
• Support transitions of care and safe discharge planning with DME and home services.
• Help members navigate benefits and care options.
• Assess environmental and psychosocial factors affecting health.
• Collaborate with multidisciplinary teams to coordinate care.
• Promote self-management by providing education and resources.
• Coordinate care activities among members, caregivers, providers, and care team.
• Document interactions, assessments, care plans, interventions, and outcomes timely.
