Sagility is seeking an experienced Senior Care Specialist (Registered Nurse) for a work-from-home seasonal opportunity. You will conduct telephonic health assessments, develop care plans, and coordinate interventions for Medicare, Medicaid, and group health plan members.

Responsibilities include remote care coordination, education, and collaboration with a multidisciplinary team to promote safe outcomes and patient well-being. Excellent communication and remote-working capability are essential.

Competenze

• RN license in state of residence.
• Compact RN license permitting practice in applicable states.
• Minimum 2 years clinical nursing experience (3+ preferred).
• Strong clinical judgment, critical thinking and problem-solving.
• Ability to work independently in a remote environment.
• Proficiency with MS Word, Excel, Outlook.
