Sagility is seeking a Senior Care Specialist (Registered Nurse) to work from home on a seasonal basis in the United States. You will conduct telephonic health assessments, build individualized care plans, and provide health coaching to Medicare/Medicaid members.

A remote role with opportunities to coordinate transitions of care and support patient education. You will collaborate with a multidisciplinary team and leverage your nursing judgment to meet member needs and improve outcomes.

Competenze

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.

Mansioni

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, and appointments.
• Help members and caregivers navigate healthcare benefits, services, and options.
• Assess factors affecting a member's health and well-being.
• Collaborate with multidisciplinary teams including social workers, dietitians, pharmacists, and providers.
• Promote self-management by providing education and resources.
• Coordinate care activities among members, caregivers, providers, and care team.
• Document interactions, assessments, care plans, interventions, and outcomes timely.
