Job summary

Sagility is seeking an experienced Registered Nurse for a seasonal, work-from-home role. You will perform telephonic health assessments, develop individualized care plans, and coach members on chronic condition management.

The position requires strong clinical judgment and the ability to coordinate care remotely. You will collaborate with a multidisciplinary team, document interactions, and support transitions of care while navigating Medicare/Medicaid programs.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Proficiency with Microsoft Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and health risks.
• Establish health coaching relationships and provide education, encouragement, and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health coordination.
• Help members navigate benefits, services, and care options.
• Assess factors affecting health and well-being; collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Coordinate care activities among members, caregivers, providers, and care team.
• Document interactions and outcomes accurately in applicable systems.
