Job summary

Sagility invites an experienced RN to join as a Senior Care Specialist in a full-time, remote role. You will conduct telephonic health assessments for Medicare/Medicaid populations, craft actionable care plans, and foster coaching relationships to support chronic condition management.

The position emphasizes independent practice in a home-based environment and coordination with a multidisciplinary care team. Starting pay is $37.00 per hour, with potential access to earned wages before payday.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.
• Ability to navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop individualized, measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support.
• Use nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate healthcare benefits and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams across care settings.
• Promote member self-management with education and resources.
