Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a remote, seasonal role. You will assess and coordinate care for members with chronic conditions, deliver education, and develop care plans by phone, supporting Medicare/Medicaid groups.
This assignment ends June 2027, with potential bonuses and incentives based on performance. Starting pay is $37 per hour, with daily pay options and a collaborative team supporting whole-person care from a home-based setup in
Competenze
- Active, unrestricted RN license in the state of residence.
- Active Compact RN license permitting practice in applicable compact states.
- Minimum of 2 years of clinical nursing experience; 3+ preferred.
- Strong clinical judgment and problem-solving skills.
- Ability to work independently in a remote environment.
- Proficient computer skills (Word, Excel, Outlook).
Mansioni
- Conduct telephonic health assessments for members.
- Develop actionable, individualized care plans.
- Provide education and support for chronic condition management.
- Coordinate transitions of care and DME/Home Health services.
- Navigate healthcare benefits and care options with members and caregivers.
- Collaborate with multidisciplinary teams to support member health outcomes.
