Sagility is seeking a Senior Care Specialist (RN) to perform telephonic assessments and build personalized care plans from a remote setting. This seasonal, work-from-home role focuses on Medicare/Medicaid and group health members, coordinating care and educating patients.

Ideal candidates hold an active RN license, strong clinical judgment, and the ability to operate across multiple systems. Starting pay is $37/hour with daily pay options.

Competenze

• Active, unrestricted RN license in home state.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum 2 years clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Microsoft Word, Excel, and Outlook.

Mansioni

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions.
• Support transitions of care and safe discharge planning with DME, home health services, and provider appointments.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams (social workers, dietitians, pharmacists, providers).
• Promote self-management by providing education and resources.
• Coordinate care activities among members, caregivers, and care team.
• Document interactions, assessments, care plans, interventions, and outcomes accurately.
