The position involves comprehensive assessments, care planning, and coordination across behavioral health and LTSS, with opportunities to influence outcomes and cost-efficient member care.

Qualifications

• Minimum 2 years healthcare experience; care management preferred.
• Active RN license in state of practice; valid driver's license.
• Knowledge of EMR and HIPAA regulations.
• Ability to travel up to 40% as required by state/contract.

Responsibilities

• Conduct comprehensive member assessments within timelines.
• Develop and implement care coordination plans with the care team.
• Perform telephonic, in-person, or home visits as needed.
• Monitor care plans and document interventions and outcomes.
• Maintain regular outreach to caseload; coordinate services across teams.
• Support integration of behavioral health and LTSS services.
• Facilitate ICT meetings and use Molina guidelines to motivate change.
• Identify barriers to care and provide assistance to members.
