Job summary

Molina Healthcare is seeking a remote RN Care Manager to support Medicare members via telephonic case management and transition of care. Washington state preference; PST hours apply. No travel, no night shifts.

You'll assess member needs, develop coordination plans with care teams, and monitor progress while promoting access to behavioral health, LTSS, and community resources. Strong RN background and HIPAA EMR familiarity required.

Qualifications

• Minimum 2 years in health care with care management or related setting.
• Active RN license in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.
• Understanding of EMR and HIPAA.
• Ability to work independently and handle high-pressure situations.
• Experience with diverse populations.

Responsibilities

• Coordinate care management activities with a multidisciplinary team to achieve outcomes.
• Complete comprehensive assessments within regulated timelines.
• Develop and implement care plans with member and care network.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor progress and adjust interventions as required.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health and LTSS services.
• Facilitate ICT meetings and collaboration.
• Provide education and motivational interviewing to support change.
• Identify barriers to care and address them.
