Job summary

Molina Healthcare is seeking a remote RN Care Manager to support Medicare members in Washington state via telephonic case management and care coordination.

The role operates on Pacific Time hours, with a preference for WA residents, and requires 2+ years in healthcare and active RN license. You will assess, plan, and monitor member care, collaborating with an interdisciplinary team to improve outcomes and cost-effectiveness.

Qualifications

• Minimum 2 years in health care, preferably care management.
• RN license active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.
• Strong EMR and HIPAA knowledge.
• Experience coordinating with multidisciplinary teams.
• Ability to work independently in a remote setting.
• Excellent time-management and written/verbal communication skills.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop care coordination plans with member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and evaluate care plans; document interventions and outcomes.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team meetings.
• Use motivational interviewing to educate and motivate change.
• Identify barriers to care and coordinate solutions.
• May provide consultation and support to peers; handle complex cases.
• May conduct medication reconciliation as needed.
• Travel 25-40% may be required depending on state/contract.
